Brexpiprazole falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.

Research in vitro shows that chelidonine, a constituent of greater celandine, inhibits cytochrome P450 2D6 (CYP2D6) enzyme activity. Theoretically, greater celandine might increase levels of drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.

Here's the gist for Rexulti (brexpiprazole). Your body relies on a liver enzyme called CYP2D6 to help break this medication down. One of the compounds in greater celandine, chelidonine, appears to slow that enzyme down. If CYP2D6 is working slower, brexpiprazole could clear out of your system less efficiently and build up higher than expected.

In real life, more brexpiprazole in your blood could mean stronger side effects, things like drowsiness, restlessness, dizziness, or muscle stiffness. I want to be honest though: this concern comes only from lab (test tube) research, not from people, so it's theoretical at this point. The bigger issue is that greater celandine itself has been tied to serious liver injury, which is reason enough to talk with your pharmacist or doctor before using it.

What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
