Dextromethorphan, Guaifenesin, Pseudophedrine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Out of the three active ingredients in this cough and cold combo, the one that matters here is dextromethorphan (the cough suppressant). Your body relies on a liver enzyme called CYP2D6 to break it down. Lab studies suggest greater celandine may block that enzyme, so in theory the dextromethorphan could clear more slowly and build up a bit. That might mean more dizziness, drowsiness, or feeling foggy or overstimulated. The guaifenesin and pseudoephedrine pieces aren't really affected by this pathway.
Keep in mind this is only test-tube evidence, not proof it happens in people, so I wouldn't lose sleep over an occasional dose. That said, greater celandine has been tied to serious liver harm, so I'd skip it and check with your pharmacist or doctor first.
