Durvalumab falls under IMMUNOSUPPRESSANTS — the group Greater Celandine interacts with.

Preliminary clinical research suggests that taking a specific semi-synthetic derivative of the greater celandine constituent chelidonine (Ukrain; not available in North America) might stimulate immune responses in cancer patients. Theoretically, taking greater celandine might decrease the effects of immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's the twist with Durvalumab (Imfinzi): it isn't a classic immunosuppressant that calms the immune system. It's actually an immune-boosting cancer drug (a checkpoint inhibitor) that takes the brakes off your immune system so it can attack the tumor. So the worry on file, that greater celandine might also stimulate the immune system, points in a different direction here. In theory, layering an immune-stimulating herb on top of an immune-stimulating drug could nudge your immune system even harder, which raises the concern for more immune-related side effects like inflammation of the lungs, gut, liver, or thyroid.

I want to be honest that this is theoretical, based on how the compounds behave, not on real studies with Durvalumab. On top of that, greater celandine has been tied to serious liver injury, which is a real problem during cancer care. Please don't add it without checking with your oncology team first.

About Durvalumab Injection

What Is Durvalumab Injection Used For?

Brand name: Imfinzi

Imfinzi is used to treat a wide range of cancers in adults. For lung cancer, it is approved for resectable non-small cell lung cancer (given before and after surgery), unresectable Stage III non-small cell lung cancer (after chemoradiation), metastatic non-small cell lung cancer, limited-stage small cell lung cancer (after chemoradiation), and extensive-stage small cell lung cancer.

Beyond lung cancer, Imfinzi is also approved for locally advanced or metastatic biliary tract cancer (cancers of the bile ducts and gallbladder), unresectable hepatocellular carcinoma (liver cancer), mismatch repair deficient (dMMR) advanced or recurrent endometrial cancer, BCG-naive high-risk non-muscle-invasive bladder cancer, muscle-invasive bladder cancer, and resectable gastric or gastroesophageal junction adenocarcinoma (stomach cancer at the junction with the esophagus). Most of these uses involve Imfinzi given alongside other chemotherapy drugs.
