Why this applies to Flupenthixol

Flupenthixol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Greater Celandine interacts with.

Research in vitro shows that chelidonine, a constituent of greater celandine, inhibits cytochrome P450 2D6 (CYP2D6) enzyme activity. Theoretically, greater celandine might increase levels of drugs metabolized by CYP2D6.
Some drugs metabolized by CYP2D6 include amitriptyline (Elavil), clozapine (Clozaril), codeine, desipramine (Norpramin), donepezil (Aricept), fentanyl (Duragesic), flecainide (Tambocor), fluoxetine (Prozac), meperidine (Demerol), methadone (Dolophine), metoprolol (Lopressor, Toprol XL), olanzapine (Zyprexa), ondansetron (Zofran), tramadol (Ultram), trazodone (Desyrel), and many others.

Here's the gist for Flupenthixol (Fluanxol), an antipsychotic your body partly clears using a liver enzyme called CYP2D6. In lab studies only, one of greater celandine's compounds (chelidonine) appears to slow that enzyme down. If that holds true in people, your flupenthixol could clear a little slower and build up, which might mean stronger effects and more side effects, things like drowsiness, dizziness, restlessness, muscle stiffness, or tremor.

I want to be honest though: this is only theoretical, based on test-tube work rather than real patients, so the actual risk is uncertain. The bigger concern with greater celandine is its link to serious liver injury, so I'd skip it. Please check with your pharmacist or doctor before combining anything new with flupenthixol.

Preliminary clinical research suggests that taking a specific semi-synthetic derivative of the greater celandine constituent chelidonine (Ukrain; not available in North America) might stimulate immune responses in cancer patients. Theoretically, taking greater celandine might decrease the effects of immunosuppressive therapy. Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), and other corticosteroids (glucocorticoids).

Here's an honest heads-up about this pairing. The interaction on file is really about immunosuppressant medicines, the kind used after a transplant or for autoimmune disease, because greater celandine might stimulate the immune system and work against them. Flupenthixol (Fluanxol) is actually an antipsychotic used for conditions like schizophrenia and depression, not an immunosuppressant, so this particular interaction doesn't truly apply to it. The match here looks like a category mislabel.

That said, my bigger worry with greater celandine is the herb itself. It has been linked to serious liver injury, and that's a real concern regardless of what else you take. So before adding it alongside your flupenthixol, please check with your pharmacist or doctor first.
