For a cancer drug like alpelisib, getting the full dose into your bloodstream really matters, so anything that lowers absorption could make it less effective at doing its job.

This is rated a moderate, possible interaction based on studies of other oral drugs (like metformin), not alpelisib specifically, so it's a reasonable caution rather than a sure thing. A simple fix is timing: take your alpelisib first, then wait at least 30 to 60 minutes before the guar gum. Please run your supplement plan by your oncology team before changing anything.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
