Here's the simple version: guar gum is a thick, gel-forming fiber. When you swallow it, it can grab onto pills in your stomach and gut and slow down how much of the medicine actually gets absorbed. With alvimopan (Entereg), that could mean less of the drug makes it into your system, so it may not work as well at helping your bowel wake up and get moving again after surgery.

The good news is the timing piece is easy. Spacing them out, taking guar gum about 30 to 60 minutes after your alvimopan, usually avoids the problem. Keep in mind alvimopan is given in a hospital setting around surgery for a short time, so always let your surgical team or pharmacist know about any fiber supplement before mixing the two.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
