Here's the simple version: guar gum is a thick, gel-forming fiber, and when you take it close to a pill, that gel can grab onto the medicine and slow down how much your gut actually soaks up. With Apremilast (Otezla), the concern is that less of your dose gets absorbed, which could make it work less well for your psoriasis, psoriatic arthritis, or Behcet's mouth ulcers. Apremilast is active on its own (not a prodrug), so this points toward a possible drop in benefit, not extra side effects.

This is a fairly general fiber effect, shown more clearly with drugs like metformin than with Apremilast specifically, so it's a reasonable caution rather than a strong warning. A practical fix is to space them out: take your Apremilast first, then your guar gum about 30 to 60 minutes later. Run this by your pharmacist before changing anything.

What Is Apremilast Used For?

Brand name: Otezla

Apremilast treats several immune-related conditions. In adults, it is used for active psoriatic arthritis (joint inflammation linked to psoriasis), plaque psoriasis in people who are candidates for light therapy or systemic treatment, and oral ulcers associated with Behçet's Disease (a rare inflammatory disorder causing mouth sores).

In children 6 years of age and older, Otezla is approved for active psoriatic arthritis and moderate to severe plaque psoriasis in patients weighing at least 20 kg. Otezla is the only brand supported by the evidence on this page; it is taken as an oral tablet.
