Here's the thing with guar gum and your balsalazide. Guar gum is a thick, gel-forming fiber. When you swallow it, it soaks up water and forms a gel in your stomach and gut. That gel can wrap around other pills and slow or lessen how much of them your body actually takes in. Studies have shown this with drugs like metformin and penicillin, so it's a real (though moderate) concern.

With balsalazide, the worry is that less of it gets absorbed properly, which could mean weaker control of your ulcerative colitis (think more diarrhea, belly pain, or rectal bleeding creeping back). To play it safe, many people space the two apart, taking guar gum 30 to 60 minutes after the medicine. Please run the timing by your pharmacist or doctor first.

What Is Balsalazide Used For?

Brand names: Colazal, Giazo

Balsalazide disodium capsules (sold as Balsalazide Disodium and Colazal) are used to treat mildly to moderately active ulcerative colitis — a condition where the lining of the colon becomes inflamed and causes symptoms like diarrhea, rectal bleeding, and abdominal pain. This use is approved for adults and for children ages 5 through 17.

The established treatment period is up to 8 weeks in children (ages 5–17) and up to 12 weeks in adults. Safety and effectiveness beyond those time frames have not been established in clinical trials.
