Here's the thing about guar gum: it swells into a thick gel in your stomach and gut, and that gel can grab onto medications and slow or reduce how much of them your body soaks up. Lomitapide is a pill you take by mouth, so it's reasonable to think guar gum could blunt how much actually gets absorbed.

For Lomitapide that means less of the drug working, which could weaken its LDL-lowering effect (this drug is the active medicine, not a prodrug, so less absorption simply means less benefit). The evidence here is moderate and comes from other oral drugs, not Lomitapide specifically, so this is a possible concern, not a certainty. A simple fix is spacing them out: take guar gum 30 to 60 minutes after your Lomitapide, and always run it by your pharmacist or doctor first.

About Lomitapide

What Is Lomitapide Used For?

Brand name: Juxtapid

Juxtapid (lomitapide) is used alongside a low-fat diet and other cholesterol-lowering treatments — including LDL apheresis (a procedure that filters LDL from the blood) where available — to lower LDL cholesterol, total cholesterol, apolipoprotein B, and non-HDL cholesterol in patients with homozygous familial hypercholesterolemia (HoFH). HoFH is a rare, inherited condition that causes extremely high LDL cholesterol levels from birth.

Juxtapid is approved for use in adults and in children aged 2 years and older with HoFH. It has not been proven safe or effective for other types of high cholesterol, including the more common heterozygous familial hypercholesterolemia (HeFH). Its effect on heart attacks, strokes, or other cardiovascular events has not been established.
