Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the gist for larotrectinib (Vitrakvi). Your body relies on a liver enzyme called CYP3A4 to break this cancer drug down. Lab studies hint that the active pieces of guggul (guggulsterones) may rev that enzyme up, meaning your system could clear larotrectinib faster than intended. Larotrectinib is the active drug itself, not a prodrug, so speeding up its breakdown could leave less of it working against your tumor.
I want to be honest about the strength of this: it's theoretical, based only on test-tube research, not real patients. Still, with a serious cancer medicine you don't want anything chipping away at how well it works. Please don't change anything on your own. Loop in your oncology team or pharmacist before adding guggul.

What Is Larotrectinib Used For?
Brand name: Vitrakvi
Vitrakvi is used to treat adults and children with solid tumors — meaning cancers in organs or tissues — that have a specific genetic change called an NTRK gene fusion. To be eligible, the tumor must be metastatic (spread to other parts of the body) or be one where surgery would likely cause serious harm, and other treatments must have stopped working or not be available.
Patients are selected for Vitrakvi based on an FDA-approved genetic test that confirms the NTRK fusion in their tumor. This approval was granted under the FDA's accelerated approval program, based on how well tumors responded to treatment; continued approval depends on results from ongoing confirmatory trials.
