Mifepristone falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.

Theoretically, guggul might reduce the effects of CYP3A4 substrates.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.

Here's the gist for Korlym (mifepristone): your liver uses an enzyme called CYP3A4 to help process this drug. Lab and animal studies hint that guggul (specifically compounds called guggulsterones) may speed up that enzyme. If that happens in people, your body could clear mifepristone faster, which might leave less of it working to block cortisol.

For someone taking this for Cushing's-related high blood sugar, weaker drug levels could mean your blood sugar control slips. I want to be honest though: this is theoretical, based only on test-tube research, not human studies. Still, since the goal here matters a lot, I'd skip guggul unless your prescriber or pharmacist gives you the green light.

Guggul is a gum resin from the Commiphora wightii tree, long used in Ayurvedic medicine for cholesterol, joint, and skin problems. What Is Mifepristone (Korlym) Used For?

Brand names: Mifeprex, Korlym

Korlym is prescribed for adults with endogenous Cushing's syndrome — a condition where the body produces too much cortisol — who also have type 2 diabetes or trouble controlling blood sugar. It works to bring those blood sugar levels under control.

It is only for blood sugar problems that are directly caused by Cushing's syndrome. It should not be used to treat ordinary type 2 diabetes unrelated to this condition.
