Why this applies to Voclosporin
Voclosporin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Guggul interacts with.
In vitro research shows that guggul constituents known as guggulsterones can induce CYP3A4.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the concern with pairing guggul and Lupkynis (voclosporin). Your body clears voclosporin using a liver enzyme called CYP3A4. Lab studies suggest the active parts of guggul (guggulsterones) may speed that enzyme up. If that happens in people, your body could clear voclosporin faster than intended, leaving less in your system.
Voclosporin isn't a prodrug, so lower levels likely mean weaker effects. That matters because this drug is carefully dosed to calm the immune attack on your kidneys in lupus nephritis. Losing some of that effect could let the disease flare. To be fair, this is based only on lab work, not human studies, so it's theoretical. Still, please check with your pharmacist or doctor before combining them, and don't change anything on your own.

Guggul is a gum resin from the Commiphora wightii tree, long used in Ayurvedic medicine for cholesterol, joint, and skin problems. About Voclosporin
What Is Voclosporin Used For?
Brand name: Lupkynis
Voclosporin, sold as the capsule Lupkynis, treats active lupus nephritis in adults. This is kidney inflammation caused by lupus.
It is always used together with a background immunosuppressive regimen, not on its own. Its safety and effectiveness have not been established with cyclophosphamide, so that combination is not recommended.
