Here's the simple version: karaya gum is a bulky fiber that swells into a gel when it hits water in your gut. Like other fiber laxatives, it can sometimes grab onto a pill you swallow around the same time and change how much of that medicine actually gets absorbed. With bosentan (Tracleer), the concern is mostly theoretical, and the absorption could in theory go down, up, or not change at all.

Since bosentan is your PAH medicine and you need a steady amount in your body, it's worth keeping things consistent. A practical habit is to take your bosentan first, then wait about 30 to 60 minutes before taking the karaya gum, and drink plenty of water with the fiber. Before mixing the two, it's smart to run it by your pharmacist or doctor.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.

What Is Bosentan Used For?

Brand name: Tracleer

Bosentan is used to treat pulmonary arterial hypertension (PAH) — high blood pressure in the arteries of the lungs. In adults, it helps improve the ability to exercise and slows how quickly the disease gets worse.

In children aged 3 years and older with idiopathic (no known cause) or congenital (present from birth) PAH, bosentan is used to lower resistance in the lung blood vessels, which is expected to improve exercise ability. Tracleer is one brand approved for both adults and pediatric patients with PAH.
