Here's the simple version: karaya gum is a fiber that swells into a thick gel in your stomach and intestines. When that gel is sitting there at the same time as a pill like fedratinib (Inrebic), it can physically trap or slow how much of the drug your body soaks up. This isn't about liver enzymes, it's just a bulky fiber getting in the way of absorption.

The concern is mostly theoretical and rated minor, but with a cancer medicine like fedratinib you don't want unpredictable absorption that could weaken its effect on your myelofibrosis. An easy fix is timing: take your fedratinib first, then wait about 30 to 60 minutes before the karaya gum. Please run this by your pharmacist or oncologist before combining them.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.

What Is Fedratinib Used For?

Brand name: Inrebic

Inrebic (fedratinib) is approved to treat adult patients with intermediate-2 or high-risk myelofibrosis (MF) — a type of bone marrow cancer that disrupts normal blood cell production. This includes primary myelofibrosis as well as secondary myelofibrosis that developed after polycythemia vera or essential thrombocythemia.

It is used both in patients who have not been treated before and in those who have previously been treated with ruxolitinib, another myelofibrosis medication.
