Here's the simple version: karaya gum is a bulky fiber that swells into a gel when it hits water in your gut. That gel can sometimes grab onto a pill like Macitentan (Opsumit) and slow down or change how much of it your body soaks up. The concern here is purely theoretical, based on how fiber works, not on real cases, so I wouldn't lose sleep over it.

Still, Macitentan is doing an important job keeping the blood vessels in your lungs open, so you don't want anything getting in the way of steady absorption. An easy fix: take your Macitentan first, then wait at least 30 to 60 minutes before your karaya gum. If you have questions, your pharmacist or doctor can help you set up the timing.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.

What Is Macitentan Used For?

Brand name: Opsumit

Macitentan tablets — sold under the brand name Opsumit — are used to treat pulmonary arterial hypertension (PAH, WHO Group I), a type of high blood pressure in the arteries of the lungs. The goal is to slow the disease from getting worse and to lower the chance of being hospitalized because of PAH.

In the key clinical study, macitentan was used in adults with mainly moderate-to-severe PAH symptoms. Patients had PAH caused by unknown or inherited causes, connective tissue disorders, or congenital heart disease with repaired shunts.
