Ifosfamide falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.

Theoretically, lemongrass might decrease the metabolism of CYP3A4 substrates.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.

Here's an important wrinkle with ifosfamide: it's actually a prodrug. Your liver's CYP3A4 enzyme has to switch it on before it can attack cancer cells. The concern on file is that lemongrass (and its lemony compound citral) might slow down CYP3A4. If that enzyme works slower, your body could activate less of the drug, which in theory might mean a weaker cancer-fighting effect. To muddy things, CYP3A4 also helps form some of ifosfamide's toxic byproducts, so the real-world result isn't simple.

Now the reassuring part: this comes only from lab and animal studies, not people, so it's theoretical. The tiny amount in cooking or a cup of tea is very unlikely to matter. Still, since this is chemotherapy, please clear any concentrated lemongrass supplement or essential oil with your oncology team first.
