Why this applies to Irinotecan

Irinotecan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.

Theoretically, lemongrass might decrease the metabolism of CYP3A4 substrates.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.

Here's the idea behind this one. Your body uses a liver enzyme called CYP3A4 to help process irinotecan. Lab and animal studies suggest lemongrass (and its lemony compound citral) may slow that enzyme down a bit. If that happened in people, more of the drug could linger in your system, which in theory could mean stronger side effects from this chemo, things like severe diarrhea, low blood counts, nausea, and feeling wiped out.

I want to be honest, though: this is only a theoretical concern from test-tube and animal work, not proven in humans, and a splash of lemongrass tea or seasoning is unlikely to matter. Concentrated supplements or the essential oil are a different story. Since irinotecan is potent, please clear any lemongrass supplement with your oncology team first.

Theoretically, lemongrass might increase the clearance and decrease the levels of glucuronidated drugs.
Animal research shows that lemongrass and its constituent citral induce uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation.

Here's the gist with irinotecan (Camptosar). Your body clears its active form, called SN-38, partly by attaching a sugar molecule to it through an enzyme called UGT. Lemongrass and its main aroma compound, citral, may speed up that very enzyme, at least in lab and animal studies. In theory, that could clear the active drug a little faster and lower its levels, which might make a carefully dosed cancer treatment less effective.

I want to be honest about the evidence: this is theoretical, based on cell and animal work, not human studies. The lemongrass you'd use in cooking or an occasional tea is unlikely to matter. Concentrated supplements or the essential oil are where I'd pause. Since irinotecan is dosed so precisely, please loop in your oncologist or pharmacist before adding any lemongrass product.

  • Irinotecan lipid complex is used in combination with other medications to treat pancreatic cancer that has spread to other parts of the body that has worsened after treatment with other chemotherapy medications. Irinotecan lipid complex is in a class of antineoplastic medications called topoisomerase I inhibitors. It works by stopping the growth of cancer cells.
