Theoretically, concomitant use might have additive hepatotoxic effects.
In case reports, patients were diagnosed with liver injury thought to be related to the use of products containing ligandrol.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: this isn't about one drug changing the other's levels in your blood. It's about two things that can both stress your liver at the same time. Ripretinib is a cancer medication that can raise liver enzymes on its own, and Ligandrol (a SARM that is not an approved supplement) has been tied to liver injury in real-world case reports. Stacking them could add up and increase the chance of liver trouble.

Watch for warning signs like yellowing of the skin or eyes, dark urine, belly pain on the upper right side, unusual tiredness, or nausea. I'll be honest, the evidence here is limited and based on anecdotal reports, so it's a caution, not a certainty. Still, please don't add Ligandrol while on Ripretinib without talking to your oncologist or pharmacist first.

“If you take ripretinib (Qinlock) for a GI stromal tumor, check this list carefully, since six of its 168 entries rate major. Ripretinib is cleared by the liver enzyme CYP3A4, and 121 of these ingredients can push its blood level up or drag it down through that pathway.”
