Lime and Venetoclax

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Venetoclax

Venetoclax falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lime interacts with.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Venclexta (venetoclax). Your body clears this medicine mainly through a liver enzyme called CYP3A4. Some lab and animal research suggests that compounds in lime juice may slow that same enzyme down a bit. If that happened in people, venetoclax could clear out more slowly and build up in your blood, which can mean stronger effects and more side effects (think low blood counts, infection risk, diarrhea, nausea, or fatigue). Venetoclax also carries a real risk of tumor lysis syndrome, so blood levels matter.

The honest part: this is only theoretical, based on animal data with high lime doses, not human studies. A squeeze of lime in your food is very unlikely to be a problem. Still, before using concentrated lime products, check with your oncology pharmacist or doctor.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

From our pharmacist’s overview on the Venetoclax page

“Venetoclax (Venclexta) is a targeted leukemia and lymphoma drug whose blood level is managed carefully, since too much invites serious side effects and too little gives the cancer room. That makes this a list to treat with genuine caution.”

  • Digestive upset
  • Skin care
  • Mood and aromatherapy
  • Flavoring and food preservation

Lime is a common citrus fruit and a good natural source of vitamin C and plant antioxidants.
