Alfentanil falls under CNS DEPRESSANTS — the group Magnolia interacts with.

Theoretically, concomitant use of large doses of magnolia bark and CNS depressants might have additive effects.
In vitro and animal research shows that constituents extracted from magnolia bark, especially honokiol and magnolol, have sedative effects. These effects may be due to the inhibition of catecholamine release and modulation of gamma-aminobutyric acid-A (GABA-A) receptors.

Here's the thing to keep in mind: alfentanil is a strong, fast-acting opioid that's usually given during surgery or short procedures in a hospital setting. It already slows down your breathing and makes you very drowsy. Magnolia bark, based on lab and animal studies, appears to have its own calming, sedative-like activity (possibly by acting on GABA pathways in the brain).

So the worry here is simply adding two calming effects together. In theory, pairing concentrated magnolia supplements with alfentanil could deepen sedation or slow breathing more than expected. I'll be honest, this is based on early research, not human studies, so it's not a sure thing. Still, since alfentanil is so powerful, please tell your anesthesia team or pharmacist about any magnolia you take.
