Marjoram and Edrophonium

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CHOLINERGIC DRUGS

Why this applies to Edrophonium

Edrophonium falls under CHOLINERGIC DRUGS — the group Marjoram interacts with.

In vitro research suggests that marjoram extract can inhibit acetylcholinesterase activity. Theoretically, using marjoram in medicinal amounts along with cholinergic drugs might have additive effects and increase the risk of cholinergic side effects.
Cholinergic drugs include bethanechol (Urecholine), donepezil (Aricept), echothiophate (Phospholine Iodide), edrophonium (Enlon, Reversol, Tensilon), neostigmine (Prostigmin), physostigmine (Antilirium), pyridostigmine (Mestinon, Regonol), succinylcholine (Anectine, Quelicin), and tacrine (Cognex).

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist. Edrophonium works by blocking acetylcholinesterase, the enzyme that breaks down acetylcholine. That lets acetylcholine build up and ramp up cholinergic activity. Lab studies hint that marjoram extract may block that same enzyme, so in theory the two could push in the same direction and stack their effects.

If that happened, you might see stronger cholinergic side effects like watery eyes, drooling, sweating, nausea, cramping, a slow heartbeat, or muscle twitching. That said, this is very weak, test-tube level evidence, and edrophonium is given in a monitored clinical setting for short tests. Sprinkling marjoram on food almost certainly won't matter, but concentrated supplements or oils are a different story. If you take marjoram supplements, mention it to your doctor or pharmacist beforehand.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Edrophonium page

“Edrophonium (Tensilon, Enlon) is a short-acting medicine given in clinics, usually to test for myasthenia gravis or reverse muscle relaxants, and none of the 22 supplements on this page rise above moderate concern. Nearly all of them act the same way edrophonium does, blocking the enzyme that breaks down acetylcholine, so pairing them…”

Marjoram
  • Digestive upset
  • Sleep and relaxation
  • Respiratory complaints (cough, congestion)
  • Menstrual discomfort
  • Aromatherapy for stress

Marjoram is a Mediterranean kitchen herb related to oregano that is widely used in food and traditional medicine for digestive, sleep, and respiratory complaints. Most of its health claims rest on traditional use and small lab or animal studies rather than strong human trials. It is generally considered safe as a food, but concentrated supplements and essential oils should be used with caution and avoided in pregnancy.
