Here's the thing to know about marshmallow root: it's loaded with mucilage, a slippery, gel-forming fiber. When that gel coats your stomach and gut, it can theoretically slow or reduce how well a swallowed pill like alpelisib (Piqray) gets absorbed. Alpelisib works as it is, so it isn't a prodrug that needs activating. The worry here is simply that less of it might get into your system, which could mean it doesn't work as well against your cancer.

I want to be honest that this is a theoretical concern based on how mucilage behaves, not something well documented with alpelisib specifically. A simple fix is spacing them out, taking marshmallow 30 to 60 minutes after your dose. Given that alpelisib is a serious cancer medicine, please run this by your oncology pharmacist or doctor first.

About Alpelisib

What Is Alpelisib Used For?

Brand name: Piqray

Alpelisib is approved for two distinct uses, each tied to a specific brand. Piqray is used in combination with another drug called fulvestrant to treat adults with hormone receptor (HR)-positive, HER2-negative, advanced or metastatic breast cancer whose tumors carry a PIK3CA mutation — and only after the cancer has progressed on or after a hormone-based treatment. A companion diagnostic test is required before starting Piqray.

Vijoice is used to treat adults and children 2 years of age and older who have severe manifestations of PIK3CA-Related Overgrowth Spectrum (PROS) — a rare condition where PIK3CA gene mutations cause abnormal tissue, vascular, and bone overgrowth — and who need systemic (whole-body) therapy. Vijoice's approval for PROS was granted under the FDA's accelerated approval pathway based on response data, and continued approval may depend on confirmatory trials.
