Why this applies to Alvimopan
Alvimopan falls under ORAL DRUGS — the group Marshmallow interacts with.
Marshmallow contains mucilage which can affect oral drug absorption. To avoid changes in absorption, take marshmallow 30-60 minutes after oral medications.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: marshmallow root is loaded with mucilage, a slippery fiber that swells into a gel in your gut. The concern is that this gel could form a coating that grabs onto an oral pill like alvimopan and slow down how much your body absorbs. If less alvimopan gets in, it may not protect your bowel as well, which could mean a slower return to normal bowel movements after surgery.
I'll be honest with you though, this is theoretical and based on how the fiber behaves, not on real-world reports with alvimopan. To stay on the safe side, separate them: take your alvimopan first, then wait 30 to 60 minutes before any marshmallow. And since alvimopan is a short hospital-period medicine, just check with your care team before adding it.

What Is Alvimopan Used For?

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.
This medication is only given during a short hospital stay and is not for long-term or home use.
