Hydroxyzine falls under Anticholinergic Drugs — the group Methylene Blue interacts with.

Concomitant use may increase anticholinergic effects and adverse effects. In a case report, a critically-ill male in his mid-60s developed a toxicological process attributed to anticholinergic syndrome following treatment with methylene blue for vasoplegia. Symptoms included anhidrosis, fever, lactic acidosis, mucosal dryness, and pupil dilation. Potential exacerbating factors included renal failure and use of fentanyl intra-operatively.

Hydroxyzine already has some anticholinergic activity, which is why it can leave you feeling drowsy, dry-mouthed, or a little foggy. Methylene blue can add to those same kinds of effects. The concern here is that stacking the two together might push things further, so you could notice more dry mouth, blurry vision, constipation, trouble urinating, a faster heartbeat, or feeling overheated because you sweat less.

I want to be honest about the strength of this: it rests mostly on a single case report in a very sick, hospitalized patient with other complicating factors, so it is a possible, not a certain, problem. Still, it is worth a quick heads-up. Before combining them, please check with your pharmacist or doctor, and don't stop either one on your own.

Its popular use as a supplement for energy, brain health, or anti-aging is not well supported by solid human evidence, and it can cause serious side effects. About Hydroxyzine

What Is Hydroxyzine Used For?

Brand names: Atarax, Vistaril, Ucerax

Hydroxyzine is used to relieve anxiety and tension related to psychoneurosis (anxiety-based mental health conditions) and as a supportive treatment when anxiety is part of another medical condition. Oral forms — including tablets, capsules, and oral solution — are used for anxiety relief and to manage itching (pruritus) caused by allergic conditions such as chronic urticaria (hives), atopic dermatitis, contact dermatitis, and histamine-driven itch. Oral forms are also used as a sedative before and after surgery or general anesthesia.

Hydroxyzine Hydrochloride intramuscular injection is used specifically when a shot is needed — for example, in acute psychiatric or emotional emergencies, acute alcohol withdrawal with anxiety symptoms or delirium tremens, control of nausea and vomiting (except during pregnancy), and as pre- and post-operative or pre- and post-partum supportive medication to reduce narcotic requirements and control nausea.
