Clomipramine falls under Anticholinergic Drugs — the group Methylene Blue interacts with.

Concomitant use may increase anticholinergic effects and adverse effects. In a case report, a critically-ill male in his mid-60s developed a toxicological process attributed to anticholinergic syndrome following treatment with methylene blue for vasoplegia. Symptoms included anhidrosis, fever, lactic acidosis, mucosal dryness, and pupil dilation. Potential exacerbating factors included renal failure and use of fentanyl intra-operatively.

Here's the thing to know if you're taking clomipramine (Anafranil) and thinking about methylene blue. Clomipramine already has strong anticholinergic properties, meaning it can cause dry mouth, blurry vision, constipation, trouble urinating, a fast heartbeat, and feeling overheated. When methylene blue is added to the mix, the concern is that these same effects could stack up and get worse.

I want to be honest about the strength of this: it comes from a single case report in a very sick, hospitalized patient with other complicating factors, so it's fairly weak, anecdotal evidence rather than a proven, everyday problem. Also, methylene blue affects serotonin, and clomipramine raises serotonin too, which is a separate reason to be cautious. Please don't start, stop, or combine these on your own. Loop in your doctor or pharmacist first.
