Ipratropium, Albuterol falls under Anticholinergic Drugs — the group Methylene Blue interacts with.

Concomitant use may increase anticholinergic effects and adverse effects. In a case report, a critically-ill male in his mid-60s developed a toxicological process attributed to anticholinergic syndrome following treatment with methylene blue for vasoplegia. Symptoms included anhidrosis, fever, lactic acidosis, mucosal dryness, and pupil dilation. Potential exacerbating factors included renal failure and use of fentanyl intra-operatively.

Combivent combines two inhaled medicines: albuterol (which opens the airways) and ipratropium, which is an anticholinergic. Methylene blue can add its own mild anticholinergic-type activity on top of that. So the concern here is stacking: two things nudging your body in the same 'drying' direction at once. With ipratropium, that could show up as things like dry mouth, blurred vision, urinary trouble, constipation, or a faster heartbeat.

Keep this in perspective. This warning rests on a single case report in a very sick hospitalized patient who had other risk factors, so the real-world evidence is weak. For someone using their inhaler at home, a big reaction is unlikely. Still, if you're considering methylene blue, please run it by your pharmacist or doctor first.
