Concomitant use may increase anticholinergic effects and adverse effects. In a case report, a critically-ill male in his mid-60s developed a toxicological process attributed to anticholinergic syndrome following treatment with methylene blue for vasoplegia. Symptoms included anhidrosis, fever, lactic acidosis, mucosal dryness, and pupil dilation. Potential exacerbating factors included renal failure and use of fentanyl intra-operatively.

Here's the situation with Promazine (Sparine) and methylene blue. Promazine is an older antipsychotic in the phenothiazine family, and one of its built-in traits is fairly strong anticholinergic activity. That's why it can already cause things like dry mouth, blurred vision, constipation, trouble urinating, and a faster heartbeat. Methylene blue can add to that anticholinergic load, so stacking the two may make those effects stronger or more likely.

Honestly, the evidence here is thin, based mostly on a single case report in a very sick hospital patient who also had kidney problems and other drugs on board. So this isn't a strong, well-proven interaction. Still, since promazine already leans anticholinergic, it's worth a quick chat with your pharmacist or doctor before combining them.
