HEPATOTOXIC DRUGS

Acetaminophen, Aspirin falls under HEPATOTOXIC DRUGS — the group Niacin interacts with.
Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: this combo product contains acetaminophen (Tylenol), which is processed by your liver and can stress it in higher doses. High-dose niacin can also be hard on the liver, especially the sustained-release kind. So the worry is that taking both together might add up and put a little extra strain on your liver. The aspirin piece is less about the liver and more about stomach irritation.
I want to be honest that this concern is theoretical, based on how these substances behave, not on strong proof that it commonly happens. Everyday vitamin amounts of niacin are very unlikely to matter. The caution really applies to concentrated, prescription-strength niacin. If you take that kind, check with your pharmacist or doctor before pairing it with this medicine.

ANTICOAGULANT/ANTIPLATELET DRUGS

Several cases of clotting factor synthesis deficiency and coagulopathy have been reported in patients taking sustained-release niacin. Also, thrombocytopenia has been reported in patients treated with niacin or niacin plus lovastatin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the deal with this combo product (Gemnisyn contains both acetaminophen and aspirin). The piece that matters here is the aspirin, since it thins the blood by reducing how well your platelets stick together. The thinking is that high-dose niacin may add a little to that blood-thinning effect, because there have been rare reports of clotting problems and low platelet counts in people on big doses of niacin.
I want to be honest with you: this is a theoretical concern based on how the body works, not something proven to happen often. Still, the realistic worry would be easier bruising or bleeding (think nosebleeds, bleeding gums, or a cut that's slow to stop). Everyday food amounts of niacin won't do this. If you're using concentrated niacin supplements, just run it by your pharmacist or doctor first.

ASPIRIN

Aspirin is often used with niacin to reduce niacin-induced flushing. Doses of 80-975 mg aspirin have been used, but 325 mg appears to be optimal. Aspirin also seems to reduce the clearance of niacin by competing for glycine conjugation. Taking aspirin 1 gram seems to reduce niacin clearance by 45%. This is probably a dose-related effect and not clinically significant with the more common aspirin dose of 325 mg.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the interesting part: this combo can actually work in your favor. The aspirin in Gemnisyn is the part that interacts with niacin. At higher aspirin doses (around 1 gram), aspirin competes for the same pathway your body uses to clear niacin, so niacin hangs around a bit longer. People sometimes use a low dose of aspirin on purpose to ease the warm, red, tingly flushing that high-dose niacin can cause.
The practical takeaway: at the everyday acetaminophen-aspirin doses in Gemnisyn, this effect is minor and usually not something to worry about. If you're taking high-dose (prescription-strength) niacin, just loop in your pharmacist or doctor so they can keep an eye on flushing and overall tolerance.
