Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Niacinamide interacts with.

Theoretically, niacinamide may have additive effects when used with anticoagulant or antiplatelet drugs, especially in patients on hemodialysis.
Several cases of thrombocytopenia have been reported for hemodialysis patients treated with niacinamide 1 gram daily. Hemodialysis patients receiving niacinamide had almost a three-fold higher risk of developing thrombocytopenia when compared with those receiving placebo.

Here's the gist for Iloprost: besides relaxing the blood vessels in your lungs, it also makes your platelets a little less sticky, so it has a mild blood-thinning side. Our file flags niacinamide because high doses (around 1 gram a day) have been linked to a drop in platelet counts, mostly in people on hemodialysis. Stacked on top of iloprost, the theory is they could both nudge bleeding risk in the same direction.

Honestly, this is a theoretical, anecdotal concern, and unlikely for most folks. Everyday vitamin B3 amounts almost certainly won't matter. Still, watch for easy bruising, nosebleeds, or unusual bleeding, and check with your pharmacist or doctor before taking a high-dose niacinamide supplement.

Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
