Caffeine (OTC drug) falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Nutmeg interacts with.
Animal research suggests that intraperitoneal injections of myristicin, a constituent of nutmeg, can induce CYP1A2.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for caffeine. Your liver uses an enzyme called CYP1A2 to break caffeine down. In some animal studies, a compound in nutmeg called myristicin seemed to speed that enzyme up. If that happened in people, your body might clear caffeine a little faster, meaning your pill could wear off sooner or feel weaker, so you might notice less of that alert, perked-up effect.
Honestly, I wouldn't lose sleep over this one. The evidence is only from lab and animal work using injected doses, not from people sprinkling nutmeg on food. Normal cooking amounts almost certainly won't change how your caffeine works. If you're taking concentrated nutmeg supplements, run it by your pharmacist or doctor first.
