Acetaminophen, Orphenadrine falls under CYTOCHROME P450 2E1 (CYP2E1) SUBSTRATES — the group Onion interacts with.

Theoretically, taking onion might increase the levels and clinical effects of drugs metabolized by CYP2E1.
Animal research shows that taking onion powder inhibits CYP2E1. However, this interaction has not been reported in humans.

Here's the short version on combining onion with this combo product (acetaminophen plus orphenadrine). One of the liver enzymes that helps process acetaminophen is called CYP2E1. In animal studies, onion powder seemed to slow that enzyme down a bit. In theory, slowing it could let a little more acetaminophen hang around. The honest truth, though, is that this has never actually been shown in people, and normal food amounts of onion are very unlikely to matter.

If you ever did notice anything with concentrated onion supplements, it would more likely be mild acetaminophen-type effects. The bigger thing to watch is your total daily acetaminophen from all sources. When in doubt, check with your pharmacist.
