Oregon Grape and Sulfamethoxazole, Trimethoprim

Why this applies to Sulfamethoxazole, Trimethoprim

Sulfamethoxazole, Trimethoprim falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Oregon Grape interacts with.

Theoretically, Oregon grape might increase serum levels of drugs metabolized by CYP2C9.
Preliminary clinical evidence suggests that berberine, a constituent of Oregon grape, can inhibit cytochrome P450 2C9 (CYP2C9).

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version for Bactrim (sulfamethoxazole with trimethoprim). The sulfamethoxazole part is partly broken down by a liver enzyme called CYP2C9. Berberine, a compound in Oregon grape, may slow that enzyme down a bit. In theory, that could let a little more sulfamethoxazole build up in your blood, which might nudge up the usual side effects like nausea, upset stomach, rash, or sun sensitivity.

I want to be honest with you though: this is mostly a theoretical concern based on limited evidence, so for most people taking a short antibiotic course it's unlikely to cause real trouble. Concentrated Oregon grape supplements matter more than tiny amounts. Still, please run it past your pharmacist or doctor before combining them.
