CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Alprazolam
Alprazolam falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the deal with these two together. Your body clears alprazolam (Xanax) using a liver enzyme called CYP3A4, and Panax ginseng can nudge that enzyme in either direction. The clearer signal we have is from a similar benzodiazepine, midazolam, where ginseng actually sped up its breakdown by a fair amount. If the same thing happens with alprazolam, your dose could get cleared a bit faster, which might leave your anxiety or panic symptoms less well controlled.
That said, this is still mostly theoretical for alprazolam specifically, so I wouldn't panic over it. Alprazolam isn't a prodrug, so it doesn't need activating. If you'd like to try ginseng, just loop in your pharmacist or doctor first so they can keep an eye on whether your medication is still working for you.

Alprazolam treats generalized anxiety disorder (GAD) and panic disorder, with or without agoraphobia, in adults. These uses are supported across multiple alprazolam products, including tablets, orally disintegrating tablets, oral solution, and extended-release tablets.
Xanax (tablet) is indicated for the acute treatment of GAD and for panic disorder. Xanax XR (extended-release tablet) and alprazolam extended-release tablets are indicated specifically for panic disorder with or without agoraphobia — not for GAD. Alprazolam orally disintegrating tablets and alprazolam oral solution are indicated for both GAD and panic disorder.
