Glasdegib falls under QT INTERVAL-PROLONGING DRUGS — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng has an additive effect with drugs that prolong the QT interval and potentially increase the risk of ventricular arrhythmias. However, research is conflicting.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.

Here's the concern with pairing Panax ginseng and glasdegib (Daurismo). Glasdegib is already known to stretch out the heart's electrical recovery time, something we call QT prolongation. When the QT interval gets too long, it can rarely trigger a dangerous irregular heartbeat. Some short-term studies suggest ginseng may also nudge the QT interval longer, so the worry is that the two together could add up.

I want to be honest though: this is largely theoretical, and the research on ginseng is mixed. Longer-term use didn't show QT changes. Still, since glasdegib already carries a heart-rhythm caution, I'd loop in your oncology team or pharmacist before adding ginseng. They may want a quick EKG check. Please don't stop your glasdegib on your own, it's an important part of your leukemia treatment.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Glasdegib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the thing with Glasdegib (Daurismo): your liver uses an enzyme called CYP3A4 to clear it out of your body. Panax ginseng can nudge that enzyme in either direction, so its effect on Glasdegib isn't a one-way street. If ginseng slows the enzyme down, more Glasdegib can build up, which may worsen side effects like muscle cramps, nausea, fatigue, hair loss, or a change in your heart rhythm (QT prolongation). If ginseng speeds the enzyme up instead, your Glasdegib level could drop, which risks the leukemia treatment not working as well.

This is mostly theoretical and based on limited data, so don't panic. But because Glasdegib is a serious cancer medicine, please loop in your oncology team or pharmacist before adding ginseng.

What Is Glasdegib Used For?

Brand name: Daurismo

Daurismo (glasdegib) is approved to treat newly-diagnosed acute myeloid leukemia (AML) — a fast-growing cancer of the blood and bone marrow. It is used specifically in adults who are 75 years of age or older, or in adults of any age whose other health conditions make intensive induction chemotherapy too risky.

Daurismo is not used alone. It is always given together with low-dose cytarabine, a separate chemotherapy medicine, as part of a 28-day treatment cycle.
