Letrozole, Ribociclib Succinate falls under QT INTERVAL-PROLONGING DRUGS — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng has an additive effect with drugs that prolong the QT interval and potentially increase the risk of ventricular arrhythmias. However, research is conflicting.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.

Here's the thing to know about your Kisqali Femara combo: ribociclib is already one of those medicines that can stretch out the heart's electrical rhythm (what we call the QT interval). That's why your doctor often checks an EKG when you start it. Panax ginseng has, in some short-term studies, also nudged the QT interval a bit longer. Stack the two together and the concern is mainly additive, meaning a slightly higher chance of an irregular heartbeat.

I'll be honest, the evidence here is mixed and largely theoretical, and longer-term ginseng use didn't show the same effect. Still, because ribociclib is the kind of drug where rhythm changes genuinely matter, I'd loop in your oncology team or pharmacist before adding ginseng rather than starting it on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Letrozole, Ribociclib Succinate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Both letrozole and ribociclib (the two medicines in your Kisqali Femara Co-Pack) are broken down by a liver enzyme called CYP3A4, and ribociclib in particular is sensitive to it. Panax ginseng can nudge that enzyme in either direction, so in theory it could either raise or lower the levels of these cancer medicines. If levels climb, you might see more side effects like nausea, tiredness, low blood counts, or heart rhythm changes (ribociclib can affect the QT interval). If levels drop, the treatment could work less well, which really matters with a breast cancer regimen.

Honestly, this is mostly theoretical and the direction isn't certain, so I wouldn't panic. But because this is serious cancer therapy, please don't add ginseng without first checking with your oncologist or pharmacist.
