Letrozole, Ribociclib Succinate falls under QT INTERVAL-PROLONGING DRUGS — the group Ephedra interacts with.

Theoretically, ephedra might have an additive effect with drugs that prolong the QT interval.
Even in healthy volunteers, EKG changes including prolonged QT interval and premature atrial contractions have been reported with ingestion of recommended doses of ephedra. Ephedra may have an additive effect with drugs that prolong the QT interval. This may increase the risk of ventricular arrhythmias.

Here's the key concern with your Kisqali plus Femara treatment: ribociclib is well known to stretch out the heart's electrical reset (called the QT interval). Ephedra, with its stimulant compounds like ephedrine, can do the same thing and also speed up the heart and raise blood pressure. Stacked together, these two effects can add up and raise the risk of a dangerous irregular heartbeat.

This isn't about ephedra changing how your medicine is broken down. It's about two things pushing the heart in the same risky direction. Since ephedra is banned in U.S. supplements and ribociclib already needs heart monitoring, I'd steer clear of it entirely. Please check with your oncology team or pharmacist before adding any stimulant product.
