Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version. Avandamet combines two diabetes medicines (metformin and rosiglitazone) that work to lower your blood sugar. Panax ginseng may also nudge blood sugar down a little on its own. So when you pair them, the worry is a stacking effect, your sugar dropping lower than you'd want.

If that happens, you might feel shaky, sweaty, dizzy, lightheaded, or unusually hungry or tired. The good news is this is rated moderate and is partly theoretical, based on early research rather than strong proof, so it's not a reason to panic. Neither drug here is a prodrug, so the direction is straightforward: ginseng simply adds to the sugar-lowering. If you'd like to try ginseng, check with your pharmacist or doctor first and keep an eye on your home glucose readings.
