Why this applies to Perampanel

Perampanel falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the thing with Perampanel (Fycompa): your liver clears it using an enzyme called CYP3A4. Panax ginseng can nudge that enzyme in either direction, so this one is hard to predict. If ginseng speeds the enzyme up, your perampanel could clear faster and work less well, which might mean less seizure protection. If it slows the enzyme down, perampanel could build up and bring on more drowsiness, dizziness, trouble walking, or mood and irritability changes.

I'll be honest, this is mostly theoretical and the human evidence is limited, so I don't want to alarm you. But because losing seizure control is a real concern, please don't add or stop ginseng without checking with your doctor or pharmacist first.

What Is Perampanel Used For?

Brand name: Fycompa

Perampanel — available as Fycompa and in generic forms — is used to treat partial-onset seizures (seizures that start in one area of the brain) in patients with epilepsy aged 4 years and older. Both the tablet and oral suspension forms carry this indication.

Perampanel tablets and oral suspension are also approved as add-on (adjunctive) therapy for primary generalized tonic-clonic seizures (grand mal seizures involving the whole brain at once) in patients with epilepsy aged 12 years and older. Fycompa oral suspension and tablet share both indications.
