Panax Ginseng and Progestin, Oral Contraceptive

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Progestin, Oral Contraceptive

Progestin, Oral Contraceptive falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Your birth control pill Slynd (drospirenone) is processed by a liver enzyme called CYP3A4, and Panax ginseng may nudge that enzyme in either direction. This isn't a strong, proven concern, it's mostly theoretical, based on a few studies where ginseng sped up clearance of one drug and slowed it for another. So we can't say for sure which way it would go for your pill.

The worry that matters most: if ginseng speeds up how fast your body clears drospirenone, the hormone level could dip and make your birth control less reliable, raising the chance of an unplanned pregnancy or some spotting. Since this pill protects you from pregnancy, I'd rather you not gamble. Please run it by your pharmacist or doctor before combining them.

From our pharmacist’s overview on the Progestin, Oral Contraceptive page

“If you take Slynd, a progestin-only birth control pill, treat this list with real care, because anything that lowers the hormone level can weaken protection against pregnancy. Slynd is cleared by the CYP3A4 enzyme, and nearly all 139 ingredients on file touch that pathway one way or another.”
