Clozapine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Peony interacts with.

Theoretically, use of peony may increase the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research shows that peony suppresses the metabolism of clozapine via weak-to-moderate inhibitory effects on CYP1A2 and CYP3A4. This effect has not been reported in humans.

Here's the gist for clozapine: your body relies mostly on a liver enzyme called CYP1A2 to break it down and clear it out. In lab studies, peony root seems to slow that enzyme down a bit. If that happened in a real person, clozapine could build up a little higher than expected, which might mean more side effects like heavy sedation, dizziness, drooling, a fast heartbeat, constipation, or low blood pressure.

I want to be honest about the strength of this, though. This has only been seen in test-tube research, not in actual patients, and the effect looked weak to moderate. Still, clozapine is a drug we watch very carefully, so please loop in your pharmacist or prescriber before adding peony rather than changing anything on your own.

Here's the gist with peony and clozapine. Your body clears clozapine mainly through liver enzymes called CYP1A2 and CYP3A4. In test-tube studies, peony seemed to slow those enzymes down a bit. If that happened in a real person, clozapine could build up and hit harder than expected, meaning more sedation, dizziness, drooling, a faster heartbeat, constipation, or that heavy, foggy feeling.

I want to be honest, though: this is only laboratory data, and it hasn't been shown in actual patients. Still, clozapine is a drug we watch very closely (it even requires regular blood monitoring), so I don't take any added wrinkle lightly. What Is Clozapine Used For?

Brand names: Clozaril, Versacloz

Clozapine is used for two specific situations. First, it is prescribed for people with severe schizophrenia who have already tried other antipsychotic medications and have not gotten adequate relief. It is reserved for this group because of its serious risks.

Second, clozapine is used to lower the chances of repeated suicidal behavior in people with schizophrenia or schizoaffective disorder who are considered to be at ongoing, chronic risk. It is not a first-choice medication — it is used when the need is serious and other options have fallen short.
