Mavacamten falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Peppermint interacts with.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking on combining peppermint with Camzyos (mavacamten). Your body clears mavacamten largely through a liver enzyme called CYP2C19. In lab dish studies, peppermint oil appears to slow that enzyme down. If that held true in people, less of the drug would get broken down, so levels could creep up. Mavacamten is an active drug (not a prodrug), so higher levels would mean a stronger effect.
Why that matters: mavacamten lowers how hard your heart squeezes, and too much can drop it too far, causing shortness of breath, dizziness, swelling, or heart failure symptoms. That said, this is only seen in a test tube and has never been reported in real patients. Everyday culinary peppermint is unlikely to matter, but check with your pharmacist before using concentrated peppermint oil capsules regularly.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection worth knowing: your body clears Camzyos (mavacamten) partly through a liver enzyme called CYP3A4. Concentrated peppermint oil may slow that enzyme down a little, so in theory mavacamten could build up and hit harder than intended. With this medicine, that matters because too much can weaken the heart's pumping (lower ejection fraction), which is exactly why it requires regular echocardiogram monitoring.
That said, this is a fairly soft concern. The evidence comes from high-dose peppermint oil supplements, and lab studies suggest the blocking effect only shows up at high concentrations. A peppermint tea or a mint candy is very unlikely to do anything. If you take peppermint oil capsules regularly, just mention it to your cardiologist or pharmacist before mixing them.

Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.
The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
