Ashwagandha + Mavacamten
Mavacamten brand name: Camzyos.
Ashwagandha has a minor interaction with Mavacamten. Theoretically, ashwagandha might decrease the levels and clinical effects of CYP3A4 substrates.

Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
Why this applies to Mavacamten
Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ashwagandha interacts with.
In vitro research shows that ashwagandha extract induces CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the situation with these two. Camzyos (mavacamten) is cleared from your body partly by a liver enzyme called CYP3A4. Some early lab research hints that ashwagandha may speed that enzyme up. If that happened in real life, your body could break down mavacamten a bit faster, which in theory could leave you with slightly lower levels and a weaker effect on your heart condition (obstructive hypertrophic cardiomyopathy).
The good news: this is only a theoretical, lab-based concern, rated minor, and we don't have human studies showing it actually matters. Still, mavacamten is a drug that needs careful dosing and regular echocardiogram monitoring, so I wouldn't add ashwagandha on your own. Please run it by your cardiologist or pharmacist first.

By severity: 3 major · 100 moderate · 40 minor.
From our pharmacist’s overview on the Mavacamten page
“Grapefruit is the first thing I would set aside for anyone taking mavacamten (Camzyos), the medicine for obstructive hypertrophic cardiomyopathy. It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”

- General well-being (adaptogen)
Ashwagandha is an Ayurvedic herb most often taken to help with stress, anxiety, and sleep, and some small studies suggest it may help, though the evidence is still limited. Talk with your pharmacist or doctor before starting it.
About Mavacamten
What Is Mavacamten Used For?
Brand name: Camzyos
Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.
The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
