Idelalisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the short version: your body uses a liver enzyme called CYP3A4 to help clear idelalisib (Zydelig). The berberine in phellodendron can mildly slow that same enzyme down. In theory, that could let idelalisib build up a bit more than expected in your blood, which may nudge up its side effects.

With idelalisib that matters because it already carries real risks, things like liver enzyme elevations, diarrhea or colitis, lung inflammation, and serious infections. I want to be honest, though: this warning is theoretical, based on lab work rather than people taking these two together. Still, since idelalisib is a serious cancer drug, please don't add phellodendron on your own. Check with your oncologist or pharmacist first so they can weigh it for your situation.

About Idelalisib

What Is Idelalisib Used For?

Brand name: Zydelig

Zydelig (idelalisib) is approved to treat relapsed chronic lymphocytic leukemia (CLL) — a type of blood cancer that has come back after prior treatment. It is used in combination with rituximab, specifically for patients whose other health conditions make rituximab alone the right choice.

Zydelig is not approved for first-line (initial) CLL treatment, and it is not approved for small lymphocytic lymphoma (SLL), follicular lymphoma (FL), or other slow-growing non-Hodgkin lymphomas, either alone or in combination with other drugs such as bendamustine and rituximab.
