Duvelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist for your duvelisib (Copiktra): your body relies on a liver enzyme called CYP3A4 to break it down and clear it out. Phellodendron contains berberine, which lab and early human studies suggest can moderately slow that same enzyme down. If that happens, duvelisib could clear more slowly and build up in your blood, which may mean stronger effects and a higher chance of its side effects, such as diarrhea, infections, low blood counts, liver enzyme changes, or lung inflammation.

I want to be honest that this is a theoretical concern based on how these compounds behave, not proven cases in people. Still, duvelisib is a serious cancer medicine with real risks, so please loop in your oncologist or pharmacist before adding phellodendron rather than starting or stopping anything on your own.

What Is Duvelisib Used For?

Brand name: Copiktra

Copiktra (duvelisib) is used in adults with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) — meaning the cancer has returned or stopped responding to treatment — after at least two prior therapies.

Copiktra is also used for relapsed or refractory follicular lymphoma (FL) after at least two prior systemic therapies. The follicular lymphoma approval is based on overall response rate and is considered an accelerated approval; continued approval may depend on results from confirmatory trials. Copiktra is not intended for use as a first- or second-line treatment for any of these cancers.
