Why this applies to Duvelisib

Duvelisib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP3A4 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the concern with combining these two. Some Acacia rigidula products have been found to contain a hidden stimulant called BMPEA, and lab studies suggest BMPEA can partly block an enzyme called CYP3A4. That same enzyme is what your body uses to clear duvelisib (Copiktra). If that pathway slows down, more duvelisib could build up in your system than intended.

For a cancer medicine like this, higher levels could mean a bigger chance of side effects such as diarrhea, infections, liver enzyme changes, rash, or lung inflammation. That said, this is only theoretical and based on test-tube data, not real patient cases. Still, since duvelisib is serious medicine and these supplements carry their own safety worries, I'd skip the Acacia product and check with your oncologist or pharmacist first.

About Duvelisib

What Is Duvelisib Used For?

Brand name: Copiktra

Copiktra (duvelisib) is used in adults with relapsed or refractory chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL) — meaning the cancer has returned or stopped responding to treatment — after at least two prior therapies.

Copiktra is also used for relapsed or refractory follicular lymphoma (FL) after at least two prior systemic therapies. The follicular lymphoma approval is based on overall response rate and is considered an accelerated approval; continued approval may depend on results from confirmatory trials. Copiktra is not intended for use as a first- or second-line treatment for any of these cancers.
