Potato and Alteplase, tPA

Here is the interaction we found between these two, with the evidence behind it.

Moderate

THROMBOLYTIC DRUGS

Why this applies to Alteplase, tPA

Alteplase, tPA falls under THROMBOLYTIC DRUGS — the group Potato interacts with.

Theoretically, concomitant use of potato may enhance the effects of thrombolytic drugs. A carboxypeptidase inhibitor isolated from potato tubers may have inhibitory effects on thrombin-activatable thrombolysis inhibitor, and thereby enhance the activity of thrombolytic agents.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist: Alteplase (Activase) is a clot-busting drug given in the hospital for things like strokes, heart attacks, or large blood clots. It works fast and hard to dissolve clots, and its main risk is bleeding. The concern with potato is purely theoretical. A compound found in potato tubers may block one of the body's natural brakes on clot breakdown, which could, in theory, make a thrombolytic like alteplase work even more strongly and raise the chance of bleeding.

That said, this idea comes only from lab studies, not real people, so I wouldn't lose sleep over it. Eating normal potatoes is not the same as a concentrated extract. Since alteplase is given under close medical watch anyway, just let your care team know about any supplements you use.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.

From our pharmacist’s overview on the Alteplase, tPA page

“Nobody plans to receive alteplase (Activase, tPA). It arrives by IV in an emergency to dissolve the clot behind a stroke or heart attack, so the supplements already in your system at that moment matter, and this list deserves real weight.”

About Potato

What is Potato?

  • Digestive complaints and heartburn
  • Minor skin irritation or burns (topical folk use)
  • Joint or muscle aches (traditional poultice)
  • Source of carbohydrates and potassium in the diet
  • Eye puffiness (cosmetic folk use)

Potato is a common food, and as a supplement it is mostly used in folk remedies such as raw potato juice for stomach upset or as a topical poultice. Solid scientific evidence for these uses is very limited. The edible tuber is safe as food, but green, sprouted, or raw potato in large amounts can contain natural toxins, so use caution and talk to your pharmacist before using potato as a remedy.
