Why this applies to Bedaquiline

Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist with bedaquiline (Sirturo) and propolis. Your body clears bedaquiline using a liver enzyme called CYP3A4. In test-tube studies, some propolis compounds appear to slow that enzyme down. If that actually happened in people, bedaquiline could build up a bit, and one thing we watch closely with this drug is its effect on heart rhythm (a change called QT prolongation), along with nausea, headache, or liver-related effects.

That said, the evidence here is weak. It comes from lab and animal studies, not from real patients, and animal research didn't show a meaningful effect. So this is more theoretical than proven. Concentrated propolis supplements are more of a concern than tiny food amounts. Because TB treatment is so important to get right, please run any propolis use past your doctor or pharmacist first.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
