Why this applies to Busulfan

Busulfan falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist for busulfan: your body partly uses a liver enzyme called CYP3A4 to break it down. In a test tube, some propolis compounds appear to slow that enzyme. If that happened in a real person, busulfan could clear a little slower and build up, and busulfan is a powerful chemotherapy drug where higher levels can mean more bone marrow suppression (low blood counts, infection risk, bleeding) and other serious effects.

The honest part is that this is theoretical. It comes from lab and animal studies, and animal research actually showed no meaningful effect. It has never been reported in people. Still, because busulfan has such a narrow safety margin, please loop in your oncology team or pharmacist before using propolis. Don't change anything on your own.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. What Is Busulfan Used For?

Busulfan is used to treat chronic myelogenous leukemia (CML), a cancer of the blood and bone marrow. The specific use depends on the form and product.

Myleran (oral tablet) is used for the palliative treatment of CML — meaning it helps control the disease and relieve symptoms. The intravenous injection forms (Busulfex and Busulfan Injection) are used in combination with another chemotherapy drug, cyclophosphamide, as a conditioning regimen to prepare the body before an allogeneic hematopoietic progenitor cell transplant (a bone marrow or stem cell transplant from a donor) for CML.
