Propolis and Glipizide, Metformin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Why this applies to Glipizide, Metformin

Glipizide, Metformin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2C9.
In vitro research shows that propolis extract can inhibit CYP2C9. However, animal research shows that propolis extract does not significantly affect CYP2C9 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2C9 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for your Metaglip, which combines glipizide (a sulfonylurea that lowers blood sugar) and metformin. The glipizide part is partly cleared by a liver enzyme called CYP2C9. In test-tube studies, propolis seemed to slow that enzyme down. If that happened in your body, glipizide could build up a bit and push your blood sugar lower than expected, which can mean shakiness, sweating, dizziness, or feeling unusually hungry or weak.

The reassuring news is that this is theoretical. Animal studies didn't show a real effect, and it's never been reported in people. Everyday or culinary amounts of propolis are very unlikely to matter, though concentrated supplement doses are less studied. If you take propolis regularly, just mention it to your pharmacist or doctor and keep an eye on your sugar readings.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.

From our pharmacist’s overview on the Glipizide, Metformin page

“Glipizide and metformin (Metaglip) already work hard to pull blood sugar down, which is exactly why this list of 307 ingredients deserves genuine vigilance rather than a shrug. Most of them, 239, can lower glucose on their own, and stacked on this pill the combined effect can tip into true hypoglycemia.”

About Propolis

What is Propolis?

Propolis
  • Cold sores and herpes lesions (topical)
  • Sore throat and mouth sores
  • Wound and skin healing
  • Immune support
  • Oral and gum health

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.
