Why this applies to Hydrocodone

Hydrocodone falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP2D6.
In vitro research shows that propolis extract can inhibit CYP2D6. However, animal research shows that propolis extract does not significantly affect CYP2D6 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that inhibit CYP2D6 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's an interesting wrinkle with hydrocodone. Your body uses an enzyme called CYP2D6 to turn hydrocodone into hydromorphone, which is actually the stronger pain-relieving form. So hydrocodone works a bit like a prodrug. Lab (test-tube) studies hint that propolis in high amounts might slow down CYP2D6. If that happened in real life, you'd convert less hydrocodone into its active form, which could mean weaker pain relief rather than more.

The good news: this is based only on lab work. Animal studies didn't show a real effect, and it has never been reported in people. So occasional propolis (a lozenge or throat spray) is very unlikely to matter. If you take concentrated propolis daily, just mention it to your pharmacist or doctor.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist for hydrocodone and propolis. Your body uses an enzyme called CYP3A4 to help break down hydrocodone. In a few test-tube studies, propolis seemed to slow that enzyme down. If that were to happen in real life, hydrocodone could clear more slowly and build up, which might mean more of its usual effects, things like drowsiness, dizziness, constipation, and in a worst case, slowed breathing.

But I want to keep this in perspective. The evidence here is weak and based only on lab and animal work, not people. Other lab studies showed no effect at all, and animal research found no real change. So this is mostly theoretical, and concentrated supplement doses would matter more than tiny amounts. Since hydrocodone is a strong opioid, it's still smart to mention any propolis use to your pharmacist or doctor first.
